In order to be accepted into the pilot and to receive the $75 stipend, you must agree to the following:

I agree: *

I am a 9-12 grade teacher (non-educators will be considered for the pilot case-by-case space affording)

I will implement the pilot with a minimum of 4 students prior to Friday, May 15

I will use the curriculum resources provided

I will complete an online teacher feedback form (<30 minutes) prior to Friday, May 15

I will provide my students a feedback form prior to Friday, May 15 and ask them to fill it out (<10 minutes) by Friday, May 22.

I will participate in one of the following 45 minute Zoom focus groups: Monday, May 18 at 10:30 a.m. CST or Monday, May 18 at 3:30 p.m. CST
